When medication absolutely must be given during school hours, certain procedures must be followed:
    1. The parents must sign the consent form for prescription medications.
    2. The physician must complete the prescription medication form.
    3. Any medication to be given during school hours must be delivered directly to the school nurse by the parent or a responsible individual. The medication must be brought to school in the original pharmaceutical dispenser and properly labeled container.
    4. An amount greater than a 30-day supply should not be stored in the school.
    5. All medication taken by students at home should be brought to the attention of the school nurse in case a reaction occurs during the school day.
    6. In the event that there is a change in the dosage or type of medication, parents are required to re-file the parent and physician form.

    This form must be completed if a student is required to receive medications at school.
